The Golden Arrow Center for Mothers
and Children began a new system for usage of the baby closet by
all clients beginning April I, 2003. As before, clients may receive
emergency diapers, food and formula once per month.
INSTRUCTIONS FOR VOUCHER
SHEET:
Clients may receive a "full" baby closet visit, which
includes maternity clothing, baby clothes and other necessities,
after they have accomplished three tasks pertinent to their own
physical and emotion health and education. Suggested "tasks"
or referrals are listed on the REFERRAL
LIST. The client is asked to bring this form to the agency they
have been referred. The client then asks a representative of that
agency to stamp (any stamp the agency uses with their name and address
is fine) and sign and date their form in the appropriate place.
The client should bring their completed voucher form to The Golden
Arrow Center for Mothers and Children after they have achieved three
(3) referral visits for their clothing needs.
A client may also volunteer nine
(9) hours at another agency or nine (9) hours at their child's school.
The director, case manager or principal must sign and date the voucher
with the number of hours volunteered and place their stamp with
their address on this form. Also, if the client is working outside
the house, this form may be substituted with three (3) consecutive
pay stubs. The pay stubs may be dropped off on the return visit
for clothing.
